Unity Hospital, located in Fridley, Minn. serves the northern metropolitan communities with a full continuum of care and services and is known for its expertise in bariatrics and obstetrical care. With 1,500 employees and 220 staffed beds, Unity provides inpatient and outpatient services to more than 107,000 patients each year. It shares its nearly 842-physician medical staff with Mercy Hospital in Coon Rapids, Minn.
Responsibilities:
- Provide care management / coordination support to a panel on cancer patients who require specialty services integrated and in support of overall plan of care through the continuum of the cancer experience.
- Primarily working with lung cancer and lung nodule patients.
- Interacts with and supports the specialty care provider and interdisciplinary care team.
- Coordination of care and services with the interdisciplinary team to ensure smooth transitions from one service to another within out hospital and clinic systems, provide patient education and enabling informed decision making, provide emotional support and refer patient to appropriate supportive services, enhance flow of information between collaborating physicians, as well as between physicians and patients.
Qualifications:
- Bachelors Degree in Nursing or other health care related field.
- Minimum 5 years clinical experience required with 2-3 years experience in oncology specialist.
- Previous care management experience preferred.
- Demonstrate sound knowledge base and actions in the decision making process.
- Demonstrate a high level of written and verbal communication skills and effectively communicate with staff, providers, patients/family and outside agencies.
- Ability to set priorities and manage stress effectively.
- Ability to identify needs in the workplace, creatively work to resolve situations and implement new strategies.
- Must be comfortable with change and respond flexibly to multiple changes. Demonstrates expert practice skills in negotiation, networking and work delegation.
- Medical Terminology required.
Pay,Benefits, Work Schedule:
- Full time (40 hours per week).
- Day shift.
- Non-contract.
