Hays Healthcare is proud to be working alongside a Hospice based in Pontefract, West Yorkshire, to assist in the recruitment of a Lymphoedema Nurse for their service.

Job Summary:

Under the direction of the Team Leader and in collaboration with the MDT, the post holder will contribute to the effective running and development of the Lymphoedema service by:

• Assess and prioritise referrals and facilitate appointments for patients.
• Carry out a full patient assessment to identify their Lymphoedema needs.
• To formulate patient centred treatment plans and provide advice and support.
• Liaise with outside agencies for continuity of care and to obtain advice.
• Ensure that patients have transport to attend clinics via the GP.
• Ensure that Lymphoedema measurements are taken and evaluated as per the care plan.
• Maintain clear, accurate, legible and up-to-date nursing documentation.

Person Specification:

Interested applicants should please take note of the following considerations prior to submitting their application:
Qualifications Training:
• Registered 1st or 2nd level nurse able to demonstrate knowledge of current nursing issues.
• Syringe driver training, venepuncture training.
Experience:
• Minimum of two years post registration experience.
• Experience of working in a Palliative Care setting.
• Knowledge of Lymphoedema patients.
