Full-Time MDS Coordinator/Nurse Manager
The Clinical Nurse Leader is responsible for the planning, implementation, and evaluation of resident centered care planning for the respective neighborhood to comply with Community Quality standards, State and Regulatory Compliance. Responsibilities include the development of all nursing service personnel, performance management, evaluation, monitoring staffing needs, policy and procedures, and communication with resident and family members. MDS responsibilities include direct and facilitate comprehensive car planning for residents in a 65 bed SNF. Perform resident physical and mental assessment. Collect data regarding problems, strengths, goals and approaches for nursing care of each resident, utilizing Triggers and RAPS to formulate a pertinent care plan. Current RN license required, 2 - 4 yrs related exp and/or training preferred. LSS offers a full benefit package including medical, dental and retirement.
