MEDICAL RECORDS CLERK

LOCATIONS: Plantation

JOB DESCRIPTION: To handle and maintain patient medical records with confidentiality, accuracy and respect at all times.  To insure that all medical information is filed correctly and up-to-date for the member’s continuity of care.

PRIMARY DUTIES AND RESPONSIBILITIES

·          Assemble new medical record charts per QM protocol.

·          Retrieve the correct charts for schedule member appointments.  Prepare chart with all necessary documentation (i.e. encounter form, progress note forms or chart initiation form, release of information forms, etc.)

·          Files loose reports in chronological order within the Medical Record.

·          Insure that all pertinent records accompany the chart prior to the member’s appointment.

·          Review all medical record paperwork to be filed to insure appropriate physician signatures and stamps, according to QM criteria.

·          Re-file all charts in their appropriate place.

·          Maintains an out-guide when a medical record is removed from the department, indicating the name of  the individual who has possession of the record.

·          Ensures that verbal requests for medical information relating to a patient will not be honored except in the case of an emergency.  When such information is requested, the Center Administrator or Primary Care Physician will be responsible for providing it.

·          Prepares encounter forms for the next day’s appointments/visits.

·          Duplicates medical record or other forms, etc. as requested.

·          Assists with large mail-outs as needed.

·          Maintains patient and business confidentiality at all times and treats patients, the public and co-workers in a professional manner.

·          Ensures that all medical records are reviewed for completeness prior to re-filing.

·          Assumes the additional duties of the Receptionist / Appointment Clerk, Switchboard Operator, or Secretary in his/her absence.

·          Performs other tasks which are within his/her capabilities as assigned.

·          Maintains document storage and removes old medical records according to the plan’s guidelines.
