Qualifications:

 

Medication Reconciliation is a priority patient safety initiative within Alberta Health Services. Reporting to the Director, Clinical Quality Improvement, Central Zone, this position develops, coordinates, facilitates and leads the implementation and evaluation of medication reconciliation initiatives across Alberta Health Services, Central Zone at the site, sector and zone level. Through collaboration and consultation this position is responsible for influencing culture change through continual representation of the quality management body of knowledge in strategic program development, influencing change management approaches in development and implementation of Central Zone Medication reconciliation to meet required organizational practice under the Accreditation Canada standards, focusing on admission, transfer and referral. The position is accountable for the on-going monitoring, reporting, evaluation and coordination of the day to day medication reconciliation processes/planning in collaboration with leaders, clinicians, coordinators and educators across the system. Develops and maintains strong working relationships with administrative and clinical leaders at the site, sector and zone level. The achievement of defined deliverables within specific timelines and within budget restrictions is a primary accountability of this role.

Expertise:


• A current health care designation and registration/membership in a professional College is required – Nursing or Pharmacy background is preferred.
• A minimum of 5 years of operational management and leadership experience in a complex healthcare environment is required.
• A Baccalaureate degree is required.
• A Masters degree in a health care field is preferred.
• The unique knowledge and skills required include:
a. A working knowledge of project management/leadership is an asset;
b. A working knowledge of quality improvement strategies is an asset including training in AIW;
c. The demonstrated ability to lead change in a complex environment based on sound principles;
d. Well developed verbal and written communication skills;
e. Well developed critical thinking skills;
f. Flexibility and the ability to adjust to changing environments or expectations;
g. Experience with computer applications (i.e. Microsoft Suite)
h. Self-directed and able to problem solve.
