Job Details: This position supervises the coordination of care for all physician referred patients, providing a vital link that ensures appropriate customer service, effective resource use and adherence to compliance and regulatory guidelines for homecare services. Responsible for development and supervision of staff to provide the highest quality of the continuum of care for patients referred to Palmetto Health Homecare as well as all other referral agencies. All employees are expected to be knowledgeable and compliant with Palmetto Health s values of compassion, dignity, excellence, integrity and teamwork. Requirements: Education: Graduate of accredited school of nursing or graduate of accredited college with BSN or higher degree or an equivalent in nursing preferred; Experience: Minimum of 5 years of Home Care experience with demonstrated record of progressive professional leadership responsibility; Number and Type of Employees Supervised: 11 registered nurses; Licensure, Registry or Certification Required: RN currently licensed in South Carolina; Special Training; N/A.
