NHRMC Home Care - Speech Therapist

Job Details:

1. Participates in the development of the Plan of Care in writing and in collaboration with the client and/or client's family and the multi-disciplinary health team.

2. Evaluates and establishes written goals based on the client's current level of functioning and potential for improvement for speech language abilities.

3. Provides documented ST teaching/counseling for the client and/or client's family.

4. Informs the physician of all changes in the client's health status in a timely manner.

5. Maintains a safe, therapeutic environment for the client; assist in establishing household management routines.

6. Maintains detailed records of ST actions, client progress and response to therapeutic interventions and teaching.

7. Documents client records in accordance with regulatory agency guidelines, health care law, and Pender Home Health policy in a timely manner; ST notes filed in the client's clinical record within 24 hours of visit.

8. Participates in client discharge; prepares discharge summaries and instructions.

9. Communicates information effectively to appropriate members of the multi-discipline health-care team to facilitate continuity of care; documents communication.

10. Provides the multi-disciplinary personnel with supervision and teaching related to ST client care.


ESSENTIAL JOB SPECIFICATIONS:

1. Education: A Master's Degree in Speech Language Pathology or Audiology.

2. Licensure/Certificate: License to practice speech Therapy in the state of North Carolina . Valid North Carolina driver's license. Vital stem certification required.

3. Experience: One year clinical experience; Home Health experience preferred.

4. Interpersonal Skills: Excellent verbal and written communication skills required. Ability to display professionalism in all situations.

5. Essential Technical/Motor Skills: Motor coordination, dexterity and color discrimination.

6. Essential Physical Requirements: This position requires a considerable amount of physical work not to exceed 75 pounds. May be required to lift supplies and move equipment. The individual must be able to bend over to perform certain duties and be able to maneuver in and out of a motor vehicle easily.

7. Essential Mental Abilities: Excellent learning ability required. Must have the ability to reason and make judgment.

8. Essential Sensory Requirements: Interpretation of environmental input requires vision and auditory skills.

9. Exposure to Hazards: Undesirable conditions (heat, cold, uncleanliness, etc.) will be encountered. Inclement weather may be encountered. This position does have exposure to infectious body fluids.

10. Other (hours of work): Monday - Friday, 8:00AM - 5:00PM, some evening hours and weekends may be required.

11. Population served: Neonates Infants Pediatrics Adolescents (12 - 17 years) Adults ( 18 - 71 years) Geriatrics ( 72 + years)

Note: This position will also see patients in Pender Memorial Hospital.
