Nurse Assessor

I have an exciting opportunity for a RGN to work as a Nurse Assessor in the community. This role is based in Middlesbrough and surrounding areas and offers full time hours, Monday to Friday. This contract will be for a minimum of three months and permanent vacancies are available for the right candidate.

You will be involved in undertaking Continuing Health Care assessments for people in their own homes, in nursing homes or as part of their discharge from hospital. As nurse assessor you will be working closely with individuals with general and complex health needs, their carers and families, a range of Providers and Health and Social Care Professionals to ensure that those who are eligible can have their package of care funded by the NHS. You will also be part of the Multi-Disciplinary Team making recommendations in respect of eligibility for NHS Continuing Health Care.

I am looking for a motivated, enthusiastic RGN’s with excellent assessment and communication skills and who enjoy working in a friendly team environment. You must be able to think on your feet and use your own initiative. Ideally you will have previous community / elderly care experience but other skills and experience will be considered.

For this role you need to be available for full time hours, able to start once you are cleared for work through Hays, have you own car, have a valid driver’s license and a valid NMC pin number. The successful candidate will be subject to an enhanced CRB check and must be able to provide references to cover the last 3 years.
