Position Summary:
This position is responsible for evaluation and coaching of patients with chronic medical conditions.
Essential Functions:
30% Performs assessment and evaluation of potential disease management patients per client contractual obligations.
30% Collects detailed information, including medical, social and financial information to aid in facilitating the appropriate treatment plan consistent with the patient’s needs. Collaborates with health care providers and ancillary service providers to validate the appropriate treatment plan and identify educational opportunities.
25% Provides ongoing education according to the evidence based guidelines evaluates treatment adherence and re evaluates the patient’s commitment to make the necessary changes.
5% Prepares various status reports for the Company and clients. Communicates authorizations to the third party administrators to facilitate payment of services being managed. May negotiate claims and evaluate medical necessity as directed.
5% Stays abreast of health care trends and changes. Recommends changes in policy or procedures to ensure the Company maintains a competitive advantage.
5% Performs miscellaneous duties as assigned.
