The Nurse-Family Partnership (NFP), a program of Public Health Solutions, is a wonderful place to work! We offer an exciting opportunity for a Nurse Home Visitor to make a real difference in the lives of first-time, low-income mothers and their babies. The NFP is a nationally recognized, evidence-based nurse home visiting program that has been rigorously tested and proven for over 25 years. Families served by the NFP enjoy significant improvements in pregnancy outcomes, parenting skills, and the family's economic self-sufficiency. Nurses visit families in their homes from early in the pregnancy until the child's second birthday.

 

Duties:

Nurse Home Visitors provide prenatal and early childhood case management through regular home visits to first-time, low-income moms and their babies. Each nurse carries a caseload of no more than 25 families (full-time.) The Nurse Home Visitor assesses the medical and social needs of the pregnant moms, and continues to do so for post-partum moms and their babies until the child's second birthday. The Nurse Home Visitor follows a prescribed process/plan of care, and monitors and documents the outcomes. The NFP program focuses on six domains:

- Personal health

- Environmental health

- Life course development

- Maternal role

- Family and friends (social support network)

- Health and human services (utilization skills)

 

This job is based in Northwest Queens

 

Qualifications:

- Licensed Registered Nurse with at least two years nursing experience in hospital or community-based setting, preferably in OB/GYN, Pediatrics, Family Practice, Maternal Child Health.

- BSN degree or enrollment in BSN program required

- Bilingual (English/Spanish) preferred

- Able to travel by personal car if needed; travel reimbursed at NYC rate.

- Enthusiastic, and committed to community health and social change.

- Be able to demonstrate respect for individuals and diversity, be non-judgmental, understanding and compassionate.

- Strong advocacy and teaching skills, organizational and time management skills.

- Able to utilize the nursing process and draw on his/her own clinical experience.

- A self-starter who can work without constant direct supervision.

- Change lives by building therapeutic relationships.
