Nurse Navigator for Breast Services Job in Huntington 11743, New York US

Full Time, Monday - Friday 8am-4pm (flexible to work weekends, if needed)

JOB SUMMARY:  The Nurse Navigator is the point of contact for the breast cancer patient and their families.  She/he is an oncology certified nurse who will work collaboratively with memebers of the health care team which may include the breast and plastic surgeons, pathologists, mammagraphers, medical oncologists, radiation oncologists, dietitians, social workers and registered nurses.
The primary function of the nurse navigator is to coordinate services and guide patients through the health care system by facilitating timely access to quality medical and psychosocial care, identifying resources, providing education and support and developing relationships with health care  and service providers.  She/he will serve as a compassionate, knowledgeable educator and effective guide to help the patient/family through the complexities of the health care system.
The nurse navigator will develop a process that includes consistent care prior to hospitalization, through all phases of the cancer experience including cancer survivorship.  The navigator will assist in coordinating appointments, serve as a liasion with community physicians, and provide information on available clinical trials and supportive services. She/he will organize breast conferences and facilitate Breast Center related meetings.
She/he will also be responsbile for maintaining data required for maintenance of certification for NAPBC, keeping and analizing quality improvement measures, and tracking patient satifcation.
This individual must have outstanding interpersonal skills, demonstrated ability to speak and write well, current knowledge of changing healthcare enviornment, creativity in establishing new program goals for this expanding role.
EDUCATION:  BSN preferred. Oncology Certified Nurse (OCN) required.  NYS license. ONS Certified Breast Care Nurse (CBCN) required within 1 year of appointment to position. ONS chemotherapy certification within 6 months of appointment. BLS certification required.
EXPERIENCE:  Two years oncology hospital experience required.  Demonstrated leadership and teaching ability.
