Judge Medical is actively seeking Nurse Practitioners for an excellent opportunity conducting in-home assessments. These opportunities are a direct result of growth and expansion of a major health plan. This is a permanent position with flexible hours for part time or full time. POSITION SUMMARY
The Regional Lead Practitioner will spend time travelling with other practitioners as well as conducting in-home health assessments on enrolled Care members. When travelling with other practitioners, the Regional Lead Practitioner activities will include providing insight, guidance, and support in this mentoring role. The Regional Lead Practitioner will complete visits and travel with other practitioners within a geographic area of responsibility.

General Responsibilities
• Conduct in-home assessments on health plan members as described in the practitioner job description. Average visit is 45-60 minutes.
• Complete all required administrative functions for program.
• Travelling with other practitioners to provide insight, guidance, and support.
• Will be expected to travel to company headquarters in Baltimore, MD to discuss program development.
• Identify needs and opportunities for improving the program.
• Assist in training new practitioners within your geographic area of responsibility.
• Communicate findings from time spent travelling with other practitioners.
• Be compliant with all HIPAA regulations and maintain security of protected health information (PHI)
• Formulate a list of current and past medical conditions using your clinical judgment and the results of your assessment of the member.
• Coordinate with clinical team for quality reviews for practitioners in geographic area.

Required Education and Experience:

•-Full-time Home Assessment practitioner with previous experience is strongly preferred.
- NP, MD, or DO board certified or eligible with an active, unrestricted license to practice in state of assignment.
- Minimum of 3 - 5 years experience in a clinical setting preferred.  

-2 years minimum of experience as a Nurse Practitioner.

-FNP and GNP preferred.

• Strong communication skills with the geriatric or Medicare population.
• Ability to navigate on the internet and work with personal computer and computer software.
