MAJOR JOB FUNCTIONS:

  1. Evidence of decline in clinical status.
  2. Evidence of non disease-specified base guidelines, i.e., PPS, Karnofsky, dependence in ADL’s.
  3. Evidence of co-morbid conditions.
  • Communicates clinical findings to the physician who is considering recertifying the patient.
  • Completes attestation statement regarding date of face-to-face encounter.
  • Performs medically reasonable and necessary patient care as needed during face-to-face encounter.
  • Submits accurate time and mileage records for all patient related activity to the Clinical Director on a bi-weekly basis.
