- Assumes responsibility for screening, clinical management, self-management education (SME) and health system navigation for patients with or at risk for chronic medical conditions who attend facilities served by MedStar Montgomery Medical Center. In collaboration with the designated program Medical Director and facility staff follows established standards, procedures and practices and collaborates with facility staff to facilitate process and clinical outcomes practicing within the scope of licensure. Primary responsibilities relate to MedStar Glycemic Initiative for inpatient and observation patients. Secondary responsibilities relate to coordination of care across the continuum working in close collaboration with MGH Home Health and the Medical Director of Glycemic Initiative. May provide CDE services within Home Health. Requirements include MSN, NP, and licensure in Maryland as Advanced Nurse Practitioner and approval by Credentials Committee; CDE required or within 6 months upon hire. Previous related experience is required.
