Institutional based Practitioner needed ASAP! Ardmore PA Contract ?" 40 hours a week ! Up to $50hr Judge Medical is currently recruiting an Institutional Based Nurse Practitioner for an excellent opportunity in both Montgomery Delaware Counties respectively! Our client is committed to providing a high level of outpatient care and believes intensified care management of the frail, elderly, and the chronic disease patient will improve the quality of care and health status of seniors. The Institutional Based Nurse Practitioner will provide general medical care and treatment to patients residing in Long Term Care facilities (LTC) that are under contract with our client. Your 2 areas of responsibility will be in the King of Prussia and Yeadon locations. You will be under the direction of a physician and will be responsible for the following: DESCRIPTION: Perform comprehensive medical evaluations per LTC facility ??s needs. Order, interpret and evaluate tests to identify and assess patient ??s health care needs. Record findings and formulate plan of care and prognosis based on patient ??s condition. Submit health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribe or recommend drugs or other forms of treatment or related therapeutic procedures. Responsible for providing medical management and collaborating with physicians. Responsible for the treatment of acute chronic and long-term care issues. Considerable Paid-Time-Off QUALIFICATIONS: Prior experience /training (at least 1 + years) as a Nurse practitioner! Licensed as a Nurse Practitioner in the state of Pennsylvania (PA). Board certification as a Geriatric nurse practitioner a plus; Adult or family is fine. Prescriptive authority a Must! Prior experience in working with Long-term Care facilities and/or Assisted living facilities is a plus. You must be ??Ok ? with driving around within the areas of KOP to Drexel Hill and vice versa so there will be some travel. Supervisory experience, organizational skills and excellent communication abilities are necessary. Knowledge of insurance/managed care processes is required, along with knowledge of appropriate scope of end of life programs. Experience with the management of Medicare and Medicaid populations.
