Chartwell, a division of UPMC Community Provider Services (CPS), provides home infusion therapy and specialty pharmacy services in Western and Central Pennsylvania, Northeastern West Virginia and Southeastern Ohio throughout a 10-county area. These services enhance UPMC's mission of excellence in patient care and extend its reach throughout the care continuum. Services include home care services, UPMC Senior Communities, ancillary services, and rehabilitation services.

 

We are currently seeking candidates to fill immediate openings for a full-time Intake Nurse and Infusion Nurse in our Nursing Department located in Robinson.

The work hours for this position are rotating shifts starting at 7:30, 8:30, and 9:30 a.m. includes 2 weekend days per month.  Responsibilities include obtaining patient referral data, coordinating care with all departments, monitoring patients progress toward admission to service communications with referral sources, community agencies and internal staff. 

Basic Qualifications:

- Registered Nurse with 1 to 3 years of home infusion/home health experience required
- Basic computer skills, Microsoft Word and Outlook required
- Strong organizational, communication and customer service skills a must

The work hours for this position are Monday through Friday 9 a.m. to 5:30 p.m., rotating weekends with on call responsibility.  Responsibilities include providing pre-discharge teaching related to infusion therapy and enteral therapy to patients and caregivers in the hospital setting, and coordinate and perform skilled nursing care and instruction in medical management for the patients in the home setting. 

Basic Qualifications: 

- Minimum of 1 year clinical experience involving infusion therapy, including patient assessment skills, and venous access skills 

- Minimum of 1 year previous specialized home health experience preferred 

- Graduate of an accredited school of nursing 

- B.S.N. preferred 

** Must have a current PA State licensure as a Registered Nurse
