For Our Early Intervention Program

Nassau and/or Suffolk County
Bilingual a plus!!
Service Coordinator Position Requirements
· Must be able to perform all duties as an Ongoing Service Coordinator by case managing all aspects of ongoing cases from onset through closure or transition.
· Demonstrate knowledge and understanding of infants and toddlers who may be eligible for early intervention services.
· Demonstrate knowledge and understanding of state and federal laws and regulations pertaining to the early intervention program.
· Demonstrate knowledge and understanding of principles of family centered services as well as the nature and scope of services available under the Early Intervention Program of Nassau and/or Suffolk County.
· Conduct home and community-based visits throughout Nassau and/or Suffolk County.
· Write detailed and accurate activity logs
Requirements:
1. All of the following Training 2000+:
q Introduction to Service Coordination
2. An individual NYSDOH approval letter indicating “Service Coordination.”
3. A Nassau County DOH certificate of attendance at “2007 Best Practice in Early Intervention” training.
A minimum of one of the following educational or service coordination experience credentials:
· 2+ years’ experience in service coordination activities or
· One year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities; or
· One year of service coordination experience and an associate’s degree in a health or human service field or
· A Bachelor’s degree in a health or human service field
