Palliative Care Social Worker
GuildNet, a rapidly growing innovative Medicaid Managed Long Term Care and Medicare Special Needs Plan for the chronically ill, disabled and blind or visually impaired population throughout the New York City region, is seeking a Palliative Care Social Worker - LMSW for its New York office.
In this telephonic Social Worker position, the professional we seek will be responsible for the assessment, consultation, coordination and care for members with progressive/terminal illnesses.
Requirements include a Master’s degree in Social Work, licensed as a LMSW, at least one year experience in Hospice or Palliative Care as well as experience working with the adult and elderly population. Excellent oral, documentation/writing skills and computer literacy a must. Experience in case management/coordination of health care services and bilingual Spanish/English preferred.
