-- Job Title: Pharmacist Job Number: LEN0000VE Description Our Clinical Pharmacist preparation and distribution of pharmaceuticals to patients to provide quality pharmaceutical care directed at appropriate medication therapy and positive outcomes. Responsibilities Include: Process physician orders to provide pharmaceutical care services by screening orders when entering into the computer profile and reviewing for appropriate therapy e.g., incompatibilities, interactions, dosing, nonformualry agents. Prepares sterile products including large volume parenterals, small volume parenterals, TPN'S, chemotherapy utilizing aseptic technique and monitoring parameters. Modifies, clarifies, and verifies medication orders through the interdisciplinary medical team by communicating with practitioners concerning therapy changes. Dispense controlled drugs following the state and federal regulations and hospital policy including proper documentation and maintenance of inventory levels as well as the Omnicell dispensing unit for controlled substances. Maintains proper inventory control by monitoring stock levels to ensure adequate drug supplies to maintain pharmaceutical services and expiration dates to ensure quality control. Performs Computer operations to enter physician orders, maintain patient profiles, generate reports, fill patient medication needs for a twenty-four hour period, repackage unit dose for the "robotic" system, understand and operate the interfacing of the Eclipsys, Peri_birth, AllScripts HMED CPOE Sysytems and utilize the dispensing unit for controlled substances (Omnicell). Performs Monthly unit inspections by completing the assigned task of inspecting their designated patient care areas and checking the controlled drugs, refrigerators, stock levels, Medication carts, expiration dating of multidose vials, medication stashes, expiration dates, and overall medication handling practices. Performs duties in the event of an emergency such as sick call coverage, vacant position coverage, workload adjustments and projects. Maintains work area by leaving area in a neat and organized manner, cleans work area routinely, follows appropriate disposal procedures. Overseeing nonprofessional staff including the pharmacy students, assistants and messengers. Qualifications BS in Pharmacy Pharmacy graduate degree preferred One to three years health-system pharmacy experience. New York State license Active New York State registration. Knowledge and experience with pharmaceutical equipment, computer systems (TRAINING TO BE DONE) and automation of dispensing functions. Verbal and written communication, interdisciplinary skills. Ability to work independently. Ability to work with a team. Task oriented. Mathematical skills. Manual dexterity for sterile product preparation.
