Pharmacy Resident
Under direct, indirect, or general supervision as appropriate for level of training proficiency, gains specialty training in various aspects of pharmacy practice. Is responsible for provision of pharmaceutical care and patient education. Participates in target drug initiatives, operations and other services and programs provided by department. Essential Functions: - Designs, recommends, monitors, and evaluates patient specific pharmacotherapy - Prevents, identifies, and resolves medication related problems to promote safe and effective use of drugs and to help ensure positive patient outcomes - Provides services to patients, care givers and professional staff to ensure appropriate use of medications - Selects from stock, measures or counts, packages and labels, and dispenses prescriptions using appropriate techniques and applicable policies and procedures while meeting commiserate competency and licensure requirements - Develops skills to provide direct oversight for functions and activities of support personnel to assure responsibilities are performed in accordance with scope of practice and applicable procedures - Participates in established pharmacist protocols as appropriate - Participates, as appropriate, in development and/or implementation of regional drug initiatives or other formulary management activities as assigned - Participates in education of pharmacy students in provision of pharmaceutical care and in activities that enhance professional growth - Participates in management of practice area - Able to demonstrate attainment of clinical pharmacist specific skills as appropriate for level of training, competency, and licensure - Provides concise, applicable, and timely response to requests for drug information from healthcare providers and patients - May perform limited physical examinations, such as blood pressure and glucose measurement, for the purpose of providing medication management services - Demonstrates knowledge, skills, and abilities necessary to provide care and/or service appropriate to age groups served - Demonstrates knowledge, skills, and abilities necessary to provide culturally sensitive care and/or service Qualifications: Basic Qualifications: - doctor of Pharmacy degree or RPH or Pharmacy Doctorate Students who are scheduled to complete their degree in 6 months from scheduled graduation date - Completion of an emergency contraception training course approved by the American Council of Pharmaceutical Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy, applicable state or local health department programs, or programs recognized by the board of pharmacy - Valid Hawaii Pharmacist license (must obtain within six months from date of hire) - Pharmacy Intern Permit from the Board of Pharmacy, Professional Vocational Licensing Division, Hawaii State Department of Commerce Consumer Affairs - Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification - Demonstrated ability to distinguish drug products - Ability to safely handle and/or prepare compounds such as chemotherapeutic agents, biologicals or other medications - Demonstrated knowledge of and skill in oral communication, written communication, customer service, interpersonal relations, and adaptability - Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications
