Position Details:
The Physical Therapist (PT) assesses, plans, implements and monitors the rehabilitative care of individuals in their home environment. The PT directs the patients participation in those activities designed to restore, reinforce and/or enhance performance; and facilitates the learning of those skills that are essential for return of function and reduces or corrects pathology in order to promote and maintain health. Essential duties include:
1. Customer Service/Employee Relations
2. Self Development/Professional Growth
3. Patient evaluation, treatment planning, discharge planning
4. Patient treatment
5. Communication/Documentation/Collaboration
Position Requirements:
Ohio Physical Therapist License
Good time management skills
Physically able to provide treatments/to assist in patient transfers and ambulation
Able to use telephone and pager
Able to read a map
Minimum 2 years experience preferred
Educational Requirements:
Graduate of an accredited Physical Therapy Program
License Requirements:
Current license by the State of Ohio to practice Physical Therapy
CPR certification within 3 months of hire
