- Works collaboratively with AgeWell Pittsburgh partners to ensure that senior adults in our community are served within the highest standards and in keeping with best practices.
- Conducts Protective Factor Screenings™ on senior adult clients in keeping with established policies and procedures.
- Ensures that client data is entered into ETO Software in accordance with established policies and procedures.
- Provides information and referral services in keeping with established policies and procedures.
- Attends monthly service provider meetings.
- Helps identify service gaps in the community and contributes to viable solutions.
- Volunteers to participate on various task forces and committees designed to enhance senior adult services and/or implement special events.
- Participates in ongoing educational programs targeted to professional audiences related to the AgeWell Pittsburgh model.
- Establishes programs and services to meet the needs of targeted senior adult populations.
- Ensures compliance with all state, federal and local regulating agencies.
- Ensures compliance with data entry requirements as needed and generates reports.
- Maintains effective working relationships with program participants.
- Monitors program budgets and ensures compliance.
- Develops and implements department Special Events, such as: Outings, Special Meal programs, etc.
- Works closely with other department personnel in creating and implementing
agency wide and inter departmental programs.
- Actively promotes and maintains agency customer service standards.
- Generates necessary reports and documentation as needed and/or required.
- Ability to communicate verbally and in writing.
- Ability to operate personal computers (hardware and software).
- Ability to work evenings, weekends and holidays, as required.
- Ability to perform other related duties, as needed and/or required.
Supervises: Interns, Part Time Employees, Volunteers
Qualifications: Bachelor’s Degree in Social Work, Human Services, Gerontology or related field, Master’s Degree preferred, and at least three years experience working with Senior Adults in a community-based setting, or equivalent education and experience. Must possess Act 33/34 Clearance and First Aid/CPR Certification
LSW or LCSW preferred
