JOB DESCRIPTION

 

Position:  Family Caregiver Support Program Manager

 

Reports to:  Director of Community Programs

 

General Description:

The Caregiver Educator assumes the primary role and responsibility for visibility and outreach and education efforts within the eldercare network, the business community and consumer groups as they pertain to the Family Caregiver Program and agency programs and initiatives related to Alzheimer’s and related dementias.

 

Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)

 

§         Arrange and conduct  educational events and other public speaking engagements for churches, senior centers, service organizations, providers, Elder Services staff or other forums and audiences on topics related to caregiver issues, Alzheimer’s and related dementias, and other topics as appropriate and requested. Participate in media events as requested for radio, cable and print media.

§         Primary responsibility for conducting outreach efforts to increase utilization of Family Caregiver services. Oversight and management of Caregiver Scholarships.

§         Co-facilitate the Merrimack Valley Alzheimer’s Partnership monthly meetings, active participation in the Lowell Alzheimer’s Focus Group, and other groups or partnerships as appropriate. Act as Elder Services’ primary liaison with the Alzheimer’s Association.

§         Coordinate the annual Merrimack Valley Alzheimer’s Conference for professionals and family caregivers.

§         Develop and implement caregiver training programs, Corporate Eldercare Programs, and produce variety of Elder Services publications for caregivers.

§         Work closely with Title III and Development Departments on website enhancements, including website content related to the Family Caregiver Program and Alzheimer’s initiatives and programs. Conduct live web-based chats/support groups as necessary and appropriate.

 

Other responsibilities:

§         Participate in agency staff, team and statewide Family Caregiver Support Program meetings

§         Working with Eldercare Advisor, maintain accurate, up-to-date  files, including maintenance of activity as required using statewide SAMS database and using standard assessment and evaluation tools outlined by the Executive Office of Elder Affairs (EOEA), and compile regular statistics and reports as required by EOEA.

§         Pursue options for developing professional relationships with providers and other caregiver based organizations.

§         Working with Eldercare Advisor, promote, develop and facilitate caregiver supports groups in areas where a need has been identified.

§         Other duties as assigned by supervisor.

 

Qualifications:

§         MSW or MA in related field with a Social Work license strongly preferred. Bachelor’s degree with five years extensive experience, preferably with Family Caregiver and Alzheimer’s issues and programs required.

§         Excellent interpersonal, written and verbal communication skills are essential

§         Experienced public speaker

§         Computer proficiency

§         Knowledge of marketing principles in the private, non-profit field a plus

§         Flexibility regarding work schedule, willingness to commit to occasional evening and weekend commitments

§         Ability to work both independently and in team settings
