MAJOR FUNCTION
Qualified individual is responsible for direct supervision and coordinates daily activities of the assigned case management staff. Additional responsibilities include planning and training personnel in the performance of case management and related services, and monitoring and conducting quality assurance activities.
ILLUSTRATIVE DUTIES
- Directly supervises case managers.
- Responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Assigns caseloads and related duties to staff.
- Coordinates activities of staff in providing services to assist clients with problems of emergency or crisis in nature.
- Supervises daily case management tasks, plans, studies and related assignments of staff.
- Continually reviews staff work affecting case management to ensure appropriate program service methods, techniques, requirements, practices and procedures, laws, rules and regulations are followed and to ensure appropriate case load work assignments making schedule/load changes as necessary.
- Checks and verifies case plan information; conducts field reviews to evaluate case manager's interviewing information and assessment techniques; and conducts desk reviews of client files completing a check list for quality assurance monitoring.
- Assists agency staff members through individual and group conferences in analyzing case problems and in improving their case management and helping skills.
- Reviews case records and evaluates performance of staff members and recommends indicated action.
- May perform case management duties as required, including maintaining a client caseload.
- May perform in-home client assessments, re-assessments and other responsibilities of case management.
- Participates in developing and implementing division administrative policy.
- Evaluates, plans and recommends to the Program Manager any necessary procedural improvements/changes used in case management plans and or activities.
- Assist with developing and writing forms, reports, correspondence and procedural manuals.
- Completes appropriate records, forms, referral information and other pertinent data as it relates to case management activities.
- Trains new employees in areas such as agency policy, department procedures, and agency or government regulations.
- Provides in-service training for experienced workers in areas such as new policies, procedures, and regulations.
- Represents agency in community or interagency activities.
- Conducts and directs staff development programs.
- Participates in the county's special care shelters program.
- Attends required meetings, required in- service, and continuing education/seminar programs as requested to promote self growth and to stay current on issues involving elder services.
- Perform related duties as required.
KNOWLEDGE, SKILLS AND ABILITIES
Must have a basic understanding of the following:
- Knowledge of program service methods, techniques and applicable processes.
- Knowledge of applicable county, state and federal rules, regulations and laws relative to social services agencies and resources.
- Knowledge of principles and practices of case management plans and studies.
- Ability to use good judgment, ability to effectively work with public, officials and other employees.
- Ability to calculate figures and amounts such as percentages, case load, and work volume.
- Ability to define, collect data, establish facts and draw valid conclusions.
- Ability to communicate effectively both orally and in writing.
- Ability to learn the organization, functions and activities of various agencies, programs and units.
- Ability to plan, train, supervise and evaluate the work of others.
- May be exposed to safety hazards and infectious diseases including HIV and Hepatitis A and B viruses.
MINIMUM QUALIFICATIONS
Graduate of an accredited four (4) year college or university with major course work in a related field and have a minimum of three (3) years related program work experience including one (1) year of supervisory experience.Must have a valid driver's license and be able to secure a valid Florida Driver's License at the time of employment within this classification.
Must be able to accurately type a minimum of 35 wpm and possess basic skills is MS Word, Excel and Outlook.
A comparable amount of training and related experience may be substituted for the minimum qualifications.
SPECIAL REQUIREMENTS
- All new Elderly Services Division employees must undergo Level 2 (FDLE) screening submitted electronically before commencing work and every 5 years thereafter and attest, upon hiring, that they meet the requirements for employment and agree to inform the employer immediately if arrested for any disqualifying offense.
- Must be able to provide own daily transportation and minimum of Personal Injury Protection (PIP) insurance required to perform daily work assignments.
- Must pass the Uniform Client Assessment Tool test within six (6) months of employment.
