Public Health Nurse I/II Bilingual Public Health Nurse I/II

This recruitment is being conducted to fill three Full-Time and two Part-Time vacancies in the Public Health Division of the County's Department of Health Services.  The vacancies vary as to whether they can be filled at either the I or II level or only at the II level, and as to whether Bilingual Spanish/English skills at the "basic" (conversational) level are preferred or required.  These details are provided below for each position.

Public Health Nurses (PHNs) provide public health nursing services, instruction, health assessment and guidance in homes, clinics, schools, community centers, and places of employment, in connection with the prevention, detection, control of diseases and the promotion of health.  The PHN I is the entry-level in which incumbents apply the necessary knowledge, skills, and abilities in learning to perform the full scope of public health nursing duties.   The PHN II is the full journey level class in the Public Health Nurse series in which incumbents provide the full range of direct public health nursing services to the community and affected individuals and families. PHN IIs also independently perform the full range of direct public health nursing duties including assessment of community needs and development of service delivery plans necessary for the promotion and delivery of health disease prevention program services.

California Children's Services (CCS)

The CCS Program provides financial assistance and coordinates health care services such as physical and occupational therapy, case management and access to specialized medical care for children with catastrophic or serious medical conditions who meet certain eligibility requirements.

The PHNs assigned to the CCS Program develop and provide case management for the CCS children and their families at the CCS office: as well as, in their homes, at clinics and schools.  They also provide family assessment to determine the need for interventions, monitor hospitalizations of CCS children and provide discharge planning.  The CCS PHNs work to ensure appropriate case management by working closely with the CCS children and families, physicians, public and school nurses and related community agencies.

Disease Control Unit 

This Unit is responsible for the investigation of outbreaks of communicable diseases, the surveillance of reportable diseases and the provision of educational materials and services to assist our community in reducing the incidence of communicable diseases.

The Disease Control Unit's PHNs collect, summarize, and interpret information relevant to communicable infectious diseases, tuberculosis, and Immunization Registry issues. They also conduct comprehensive interviews, contact investigations, and surveys to identify need or problem and then work to identify internal and external issues that may impact delivery of essential services.  The PHNs also help to facilitate collaboration with groups to problem solve and/or implement solutions to service gaps.

Maternal Child Health (MCH) Field Nursing Unit

The Maternal Child Health (MCH) Field Nursing program is a home visiting program providing PHN resource and assessments and case management to low income, high risk pregnant and parenting women in this County.  The primary functions of the home visiting PHNs are to link clients to medical and financial resources, to prepare women for the birth of a baby, and to monitor and assess the health outcomes for both mother and baby. The goal of the program is optimal health outcomes for both mother and baby.

The PHN provides case management including nursing assessment, diagnosis, planning and guidance to pregnant and/or parenting women in their homes. This population faces multiple challenges related to the health and wellness of the mother and the care of medically fragile infants and children. In addition, the PHN is responsible to develop and maintain relationships with community resources to support client referrals.
 

MCH Field Nursing Unit Vacancy: One Part-Time vacancy which can be filled at either the I or II level and for which "basic" Bilingual Spanish/English skills are preferred.

Nurse-Family Partnership (NFP)

This evidence-based nurse home visiting program serves low-income, first-time mothers. Visits begin during the first or second trimester of pregnancy and continue until the child's second birthday. Nurse-Family Partnership's outcomes include long-term family improvements in health, education, and economic self-sufficiency.

The NFP's PHNs work with the family on developing healthy behaviors that will improve prenatal health, parenting guidance and support and to develop a plan for economic self sufficiency for the future.  They provide case management including nursing assessment, diagnosis, planning and guidance to pregnant and/or parenting women in their homes, while maintaining an active caseload of up to 25 clients.  The PHN builds and models a strength-based client-centered culture and participates in outreach, marketing and promoting public awareness of the program within the community.

Tuberculosis (TB) Control Program

The TB Program offers a number of services to assist both patients and health providers, including:

The PHNs assigned to this Program work exclusively with TB case management and contact investigations; assist in the development, implementation, and evaluation of TB Control. They work to ensure that each patient is educated about TB and its treatment, that therapy is appropriate and completed, and that contacts are examined and appropriate referrals are provided.  

TB Control Unit Vacancy: One Full-Time vacancy which can be filled at either the I or II level and for which "basic" Spanish/English bilingual skills are required.

"Basic" (Conversational) Bilingual Spanish/English Skills

As noted for each vacancy above, a sufficient level of Spanish/English bilingual skills to allow the PHN to converse proficiently in both languages (i.e., "basic" bilingual skills) is either required or preferred. An examination will be conducted prior to employment to confirm the skill level in this area. Bilingual premium pay is $.90 per hour.


Minimum Qualifications:
Public Health Nurse I  II

Special License:  Possession of a California State Public Health Nursing Certificate. (NOTE: The minimum qualifications required for a California State Certificate in Public Health Nursing are a current California License as a Registered Nurse and a Baccalaureate Degree from an accredited college or university whose program includes required preparation in Public Health Nursing. Candidates who have applied for their PHN Certificate may apply but will need to provide a current PHN Certificate prior to being selected for the position.)

License: Possession of a valid driver's license at the appropriate level including special endorsements, as required by the State of California, may be required depending upon assignment to perform the essential job functions of the position.

Working Conditions:   Duties may require exposure to hostile, emotionally disturbed or mentally ill persons and/or confrontational interpersonal situations; intermittently sit at a desk for long periods of time while reviewing patient charts and histories and preparing patient charts and reports; intermittently walk, stand, bend, climb and reach while making field visits, conducting clinics or making educational presentations or demonstrations; twist to reach equipment surrounding desk or treatment areas; perform simple grasping and fine manipulation; use telephone and write or use a keyboard to communicate through written means; see and hear with sufficient acuity to examine and assess patient conditions; lift moderate weight; may work in dangerous, high crime areas of the community; and/or standby, callback and work during non-routine hours.
