Health care Adapt of Illinois has positions open for highly motivated QUALIFIED MENTAL HEALTH PRACTITIONERS in a nursing home based psychiatric rehabilitation program located in Cobden, Illinois providing quality psychiatric rehabilitative and case management services to adult and geriatric persons with severe mental illness. ** Conducting individual and group services ** Advocacy work ** Case management ** Facilitating community placement ** Accurate documentation of services provided **Minimum of a Master's degree in Psychology, Social Work or other health/human services a must. **Interest in learning about and working with persons with severe mental illness **Excellent written and verbal communication skills **Professional appearance and demeanor **Flexibility in scheduled work hours and ability to work 4 10-hour days per week including rotating weekends and holidays.
