Reg/Case Manager CD Facility: Our Lady of the Resurrection Medical Center, Chicago, IL Department: Continuum of Care Schedule: Registry Shift: PM shift Hours: PRN: 4:30pm to 8:30pm; weekend rotation Job Details: Licensure Required Experience is required REGISTRY CASE MANAGER-CLINICAL DISTINCTION (Registered Nurse) Our Lady of the Resurrection Medical Center 5645 W. Addison (Addison Central) Chicago, IL This position is registry or PRN, so there are no benefits. Hours: 4:30pm to 8:30pm Under the general supervision of the Manager, nurses in the Case Manager role provide clinically-based case management to support the delivery of effective and efficient patient care. Paces cases from physiological and economic perspectives. Has overall accountability for the utilization management and transition management for patients within the assigned caseload. Partners with Social Workers and collaborates with other health care team members to identify appropriate utilization of resources and to ensure reimbursement. Utilizes criteria to confirm medical necessity for admission and continued stay. With the patient, family and health care team, creates a discharge plan appropriate to the patient's needs and resources. This position requires an Associate's degree in Nursing. Bachelor's degree preferred. Current IL RN licensure required. At least 1-2 years of recent acute care hospital case management experience required, working with clinical areas such as Med/Surg, Telemetry, Critical Care. Must be versed in InterQual criteria. Experience with MIDAS preferred.
