Overview:         

The Home Care Registered Nurse functions as part of the interdisciplinary case management team and participates in service planning for home care consumers.  Responsibilities include clinical assessments of clients to determine their medical eligibility for programs funded by the Executive Office of Elder Affairs and the Division of Medical Assistance. The nurse also does in home nursing assessments to determine the personal care needs of home care clients.

 

Primary Responsibilities:

  1. Does nursing assessments and develops care plans for clients referred for personal care and reassesses as needed.
  2. Participates in the case management peer review of client records and provides consultation to case managers regarding medical and nursing issues, insurance reimbursement and community resources.
  3. Collaborates with case managers to promote cost effective service plans and to determine need and eligibility for special programs funded by EOEA such as Choices and Enhanced Community Options Program (ECOP).
  4. Participates in the frail elder Waiver program by consulting with case managers and screening appropriate clients.
  5. Reviews the initial assessment and service plan for all new home care clients and makes recommendations related to medical needs and special programs.
  6. Does clinical assessments to screen home care and community clients in the BSHC areas for programs and services funded by DMA through EOEA.
  7. Provides technical assistance and training in the screening process to hospitals, certified home health agencies, nursing facilities, ADH and PACE programs such as Adult Day Health, PACE and nursing facility.
  8. Develops and implements in-service programs on medical issues for staff.
  9. Maintains collaborative relationships with agencies and institutions that serve BSHC clients.

 

Qualifications:

  1. Graduate from a school of nursing accredited by the National League for Nursing (B.S. preferred)
  2. A valid Registered Nurse’s license from the Commonwealth of Massachusetts.
  3. Certificate in Community Health or Gerontological Nursing preferred.
  4. Minimum three to five years clinical experience.
  5. Knowledge of the home care system and community resources proffered.
  6. Ability to communicate effectively and work well with other professionals.
