We are seeking a Case Manager

Job Purpose:

The case manager is responsible for the internal process of managing and coordinating all the
major aspects of the patient’s care, and communicating the relevant aspects of the rehabilitation process to all necessary parties. This process involves assisting in the pre-admission determinations, coordination of necessary and appropriate services, assuring financial reimbursement for services, promoting adequate documentation, and effectively communicating with the involving all parties affected by the rehabilitation process. The case manager will be responsible for assessing the psychosocial, medical, and fiscal needs of all pediatric, young adults, adult and geriatric patients and families, discharge planning and referrals.

Qualifications:

1. Masters degree, Bachelor’s degree or (RN) in nursing, social work and allied health
services.
2. Experience in rehabilitation and/or healthcare preferred.
3. Excellent oral and written communication skills.
4. Strong organizational and interpersonal skills.
5. Negotiation skills.
6. Ability to work autonomously.
7. Good hearing and sight to adequately communicate with patients, family, staff and third
party payors and their representatives.
8. When the position is held by a nurse, availability to offer clinical case consultation on
nursing issues.
9. Must be a Registered Nurse or Medical Social Worker
10. Discharge planning Required

Physical Requirements:

1. Light to moderate physical activity, independent mobility.
2. Lifting and carrying objects up to 20 pounds on a routine basis.
3. Practices good body mechanics, may occasionally be required to assist in moving
patients.
