Registered Nurse Hospice Intake Schedule : Full-time Shift : Day Hourly Rate : $29.11 + Depending on Experience Job Category : Hospice Nursing Location : California-Torrance Providence is calling a Hospice Nurse to Providence TrinityCare Hospice in Torrance CA. Providence TrinityCare Hospice, established in 1977, is the oldest and largest non-profit hospice in Southern California and the first to be licensed by the State of California. Caring for people in Los Angeles and Orange Counties with professional teams in Torrance, Van Nuys and Cerritos, Providence TrinityCare Hospice is Medicare Certified and CHAP (Community Health Accreditation Program) accredited. We also provide the only specialized pediatric hospice service in Los Angeles County. As the only pediatric hospice in Los Angeles County, Providence TrinityKids Care understands the impact a seriously ill child can have on the life of a family. A specialized division of TrinityCare, TrinityKids Care helps children with life-limiting illnesses leave the hospital in order to spend as much time as possible at home with their families. We are seeking a Hospice Nurse to work with our TrinityCare Hospice IntakeTeam. In this position you will: The Intake Nurse, under the supervision of the Clinical Nurse Supervisor-Intake, acts as a contact for information and arrangement of all TrinityCare Hospice services from physicians, HMOs, discharge planners and other outside referral sources. The Intake Nurse provides access for new patients to TrinityCare Hospice services. Required Qualifications: Current California Registered Nurse (RN) license. Current California drivers license with an automobile that is in good working order and insured in accordance with state requirements. Current BLS sponsored by the American Heart Association for Healthcare Provider. Minimum 1 year of professional nursing experience in hospice. Demonstrates knowledge of home care products and services. Demonstrates effective and diplomatic telephone communication skills. Demonstrates a working knowledge of computer applications including Microsoft Office and e-mail. Must be a California licensed driver with an automobile that is in good working order and insured in accordance with state and/or LCMHS/TrinityCare Hospice requirements. Preferred Qualifications: Nursing experience in hospice, palliative care, or home health. CHPN certification is preferred.
