Registered Nurse – On Call

As a On Call Registered Nurse, you will:
•Conduct services during on-call hours. Ensure that services are consistent with Company policies, service standards and applicable laws for assigned area.
•Respond to/triage calls during on-call hours in a timely manner and document all telephone contacts, patient care observations and interventions. Ensure the smooth transition to and from office staff and after on-call hours.
•Assure that interim (verbal) orders obtained from the physician are accurately documented and communicated for implementation.
•Perform initial evaluation of the appropriateness of referrals and coordinate the set up of services with the appropriate staff. Document demographic/clinical/payor information during on-call hours.
•Provides care and emotional support to the actively dying patient. Demonstrates appropriate notification of hospice team upon death.
•Complete all personal care and assist with all activities of daily living to provide comfort for patient per appropriate nursing interventions.
•Complete physical assessment including, but not limited to, vital signs, all body systems, intake and output, weight, etc.
•Educates patient/caregiver on all medications regarding indications, dose, route, time, and side effects per written and verbal instruction. Also, educates patient/caregiver on pain related to terminal illness (scale of 0 to 10, sustained medication, break-through dosing, when to notify nurse for uncontrolled pain).
•Follows appropriate procedure for refilling of medication and supplies, application of oxygen via nasal cannula/mask with oxygenator, E-tank, or oxylite system, oral pharyngeal suctioning, insertion of straight and indwelling catheters and care of indwelling catheter, of nasogastric and gastrostomy tubes, and phlebotomy technique and processing of blood specimens per protocol.
•Completes initial history and physical assessment. Prepares admission documentation and distributes appropriate paperwork to office personnel.
•Notifies physician of patient's admission to the Company.
•Evaluates patient’s condition, symptom control and any other problem relating to patient/family/caregiver (pfc) on admission and each subsequent visit.
•Prepares and implements patient’s plan of care with the cooperation of other hospice team members. Reviews plan of care bi-weekly.
•Exemplifies knowledge of and abides by all Hospice philosophy, policies and procedures, and federal, state, and nurse practice laws.

Qualifications
•Diploma, Associate or Baccalaureate Degree in Nursing
•Prefer a minimum of two (2) years experience as a Registered Nurse, preferably in the home health field.
•Current unrestricted licensure to practice as a Registered Nurse in the state(s) to which assigned and current CPR certification
•Good organizational and communication skills
•Ability to function with minimal supervision
•Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations.
