Req # (optional) 88265BR Explore a Career In Home Health Job Description Assumes primary responsibility for a patient caseload. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Conducts initial assessment and establishes plan for treatment to obtain thorough physical, psychosocial and environment data; establishes and follows a care plan which identifies problems, (nursing diagnosis) and evaluates outcomes within a certain time frame. Communicates with physicians and other team members to confirm, correlate and update patient plan of treatment and provides physician with a status report on a regular basis. Provides on-going assessment of patient response to treatments, medications and teaching and applies interventions as appropriate. Updates and revises care plan as indicated. May delegate this responsibility to staff nurse. Utilizes Care Management Initiative material consistently where appropriate and available. Schedules case conferences with manager and other members of the interdisciplinary care team on an as needed basis to discuss case management issues. Instructs and supervises paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations. Performs Home Health Aide supervisory visits. Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge. Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines. Communicates problems to physicians and manager and clearly states plans actions and goals so that changes are reflected as they occur. Participates in Continuous Quality Improvement activities and assures that quality and compliance standards are not compromised. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
