Registered Nurse-Home Healhcare-Per Diem
Reports Directly to: Clinical Manager
Position Summary
Provides professional nursing care visits to patients. Collaborates with the patient’s case manager and other members of the patient care team. Independently provides and documents patient’s visits. Covers for the case manager when appropriate.
Experience and Qualifications
Licensed as a registered professional nurse in the state. Graduate of an accredited school of nursing. Associate’s degree required. Possesses a minimum of one-year nursing experience.
Competency/Skill Requirements
Excellent written and verbal communications skills. Able to utilize the telephone for internal and external communications. Knowledgeable of nursing assessment skills. IV certified. CPR certified. Phlebotomy trained. CPR certified. Competent in the provision of nursing care.
Key Responsibilities and Duties
1. Provides home care nursing visits as assigned.
2. Submits completed documentation of the home care visit within the established time parameters.
3. Participates in the quality improvement and utilization review activities of the organization.
4. Attends multidisciplinary patient care conferences as requested.
5. Attends staff meetings and inservices.
6. Instructs patients and caregivers according to the plan of care.
7. Admits patients and develops patient plan of care adhering to the Medicare criteria for home care provision.
8. Makes nursing assessments and reports significant changes in the patient’s condition to the appropriate team members.
9. Communicates to physicians any changes in the patient’s condition.
10. Obtains physician orders for changing patient care needs.
11. Revises the plan of care based upon assessment findings.
12. Demonstrates a high degree of commitment to customer service and quality care provision.
13. Exhibits good written and verbal communications.
14. Refers patients to community resources as appropriate.
15. Maintains accurate and current drug profiles for patients assigned.
16. Recommends discharge of patients as appropriate.
17. Completes other duties as assigned or requested.
