Residential Case Manager

RESIDENTIAL CASE MANAGER
Provide DIRECT CARE support to individuals living in a community residential setting.
Provide assistance and transportation for community activities such as scheduling and attending social functions, medical appointments, meetings, and other community events.
Develop skills such as self-care, communication, independence, safety and other activities of daily living.
General household tasks such as cooking, cleaning, shovel snow, laundry etc.
Perform assistance with personal care functions such as showers, dressing, toileting, feeding etc.
Develop and implement strategies for individual support plans. Support their dreams, interests and life goals.
Develop and implement behavior plan intervention strategies
Write documentation, reports, communication logs etc.
Dispense medication, maintain documentation, monitor health and changes in support needs.
Serve as a liaison with team members, family, employer, guardian, and medical providers representing RTR in a professional and positive manner.

Requirements: Must be at least 18 years old, maintain a valid driver’s license, minimum high school grad/GED. Driving record, CORI and references will be reviewed.
