RN - Renal Care Manager

This position will have responsibility for design and development of systems, processes, and communications to support the mission of Nephrology. To plan, direct, and oversee clinical service delivery in individual designated programs, to assure high quality, sustainable nephrology clinical outcome Essential Functions: - Maintains quality of care in individual programs by developing and managing quality initiatives; designation of quality measurements and regulatory reporting of quality outcomes - Serves as a liaison between physicians, regulatory bodies, vendors, and support departments (i.e. DME, pharmacy, laboratory, quality management, utilization review, member relations) - Serves as primary representative of the individual renal program in the Northwest Region in partnership with assigned nephrologists - Research the nephrology healthcare environment for innovative concepts and best practices, assimilate and comprehend the evidence, and develop practical applications for the program. - Responsible for planning and implementation of program activities and takes independent action and accountability for achieving successful results Qualifications: Basic Qualifications: - Clinical experience with renal patients; HMO's; program management or supervisory background - Valid Oregon and Washington RN license - Valid driver's license - Case management principles, including population based management, systems framework for organizational assessment and problem solving - Excellent written/verbal communications; interpersonal skills - Ability to work with multi-disciplinary team within complex organizational structure - Ability to work independently in an unstructured environment with minimal supervision Preferred Qualifications: - Coordination of patient care within a managed care system - Spreadsheet/database/word processing skills, small group - Coordination of patient care within managed care systems Salary Range: $86,050 - $111,750
