JOB SUMMARY:
Provides skilled and compassionate education to patients, families, other health care providers and the community on the hospice benefit and philosophy.
Coordinate all activities of the admission process from point of referral to completion of paper work and transfer to the Case Manager.
MAJOR JOB FUNCTIONS:
- Completes the admission process in accordance with approved policies and procedures.
- Recommends improvements in the process to supervisor by recommending changes in policies and procedures.
- Assesses patient physical, psychological, social, and environmental status; identifies problems and appropriate interventions and initiates the Plan of Care based on these findings.
- Promotes SouthernCare and the hospice philosophy in actions and words in all activities.
- Records observations accurately and concisely and maintains patient’s medical record in accordance with agency standards.
- Plans nursing interventions to meet identified patient’s physical psychological, social, and environmental needs.
- Participates in on-call rotation in office locations with (1) Case Manager. The Admissions Coordinator is not required to participate in the on-call rotation after the 2nd Case Manager begins work.
- Participates in the Care Team meetings and all staff meetings.
- Knowledgeable regarding Hospice Medicare, Hospice Medicaid, Medicaid Waiver, other payer source, and their implications for patient care.
REQUIREMENTS:
- Graduate of an approved school of professional nursing. Licensed as a registered nurse in the State of Employment.
- Prior home health or hospice experience or one (1) year of recent medical/surgical experience or skilled nursing facility experience.
- Must be able to maintain patient confidentiality at all times and be able to abide by HIPAA rules and regulations.
- Must be proficient with computers, and hand held technical devices such as Smart Phones.
