Description:
The RN Case Manager is responsible for the coordination of the plan of care among all members of the health care team. In coordinating the plan of care, the Case Manager is accountable for the functions of Utilization Review, Discharge Planning, Continuum of Care, and Resource Management in meeting appropriate, effective, and efficient patient outcomes. This individual maintains a current knowledge of reimbursement/insurance strategies related to admission status, appropriateness of setting and continued stay requirements. They must also be able to utilize UR criteria (InterQual) in the daily management of patients in both the inpatient and outpatient setting. Finally, the Case Manager represents the department/hospital in its Continuum of Care relationships with discharge settings (HHC, SNF, etc).
Qualifications:
- Florida RN license required.
- Minimum 5 years of clinical nursing experience.
- Emergency Room experience is preferred
- Knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules preferred.
- Customer service abilities including effective listening skills.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision meeting firm deadlines.
- Assertive and persuasive communication skills, action oriented.
