RN Case Manager Job

Job Description: Provides direct and indirect skilled nursing care to homecare and hospice patients. Communicates with physicians and team members about changes in patient?s clinical condition. Is able to respond efficiently and appropriately to changes in patient condition and perform ordered interventions. Is able to perform general nursing duties. Performs case management for patients, which includes supervision and delegation to LPN?s, HHA?s, PCW?s and Volunteers. Maintains adequate number of in-service hours per year by attending Professional Staff meetings and other educational in-service/workshops. Serves on agency clinical committee. Basic Qualifications: Homecare/Hospice experience preferred, or a minimum of one year clinical experience in a health care setting working in the role of Registered Nurse or Licensed Practical Nurse. Ability to exercise independent judgment. Reliable transportation, and acceptable driving record as defined by policy #2250 ?Motor Vehicle Requirements?. Able to meet synching requirement by obtaining the appropriate supported high speed internet access at home or the ability to obtain upon hire. Basic computer skills and ability to use other technological devices required for the position. (i.e. cell phone, laptop computer, etc?). Able to maintain a steady level of productivity, a self-starter, compatible and supportive, a team player. Ability to ensure patient confidentiality. Ability to accept the philosophy, purpose, and ideas of the organization. Dependability, flexibility, willing to work irregular and/or less favorable hours (on-call and weekend hours), willing and able to drive an automobile in all types of weather (reliable automobile), able to meet synching requirement by obtaining the appropriate supported high speed internet access at home or the ability to obtain upon hire. License or Certification: Active Wisconsin RN license or eligibility to practice nursing in the State of Wisconsin.Current CPR certification or ability to become certified. Valid driver?s license, Benefit Eligible: Yes Exemption Status: Non-Exempt Hours/Pay Period: Full-time, 80 hours per pay period Schedule Details: Days and hours vary with on-call rotation Compensation Detail: Education, experience, and tenure may be considered along with internal equity when job offers are extended.
