Care at Home is seeking a part time or fulltime, experienced RN to join our office team. This position is responsible for working collaboratively with the patient/family, the physician, healthcare providers, payers, and others to develop and implement a plan that meets the individual's needs and goals for optimal health, wellness, and self-care. Prior experience with Medicare patients/OASIS will be extremely helpful to the individual in this position as well.

We are seeking someone who is passionate about being part of a team of caregivers working toward providing the highest quality of care for our clients. A positive, can-do attitude is a must for this position. Attention to detail is also a must as coordinating the details of multiple patients in varying stages of care is the primary function of this role.
This position requires a valid RN license and two years of recent acute/sub-acute clinical experience. One-year of prior case management in a Medicare organization and discharge planning experience is preferred. A Bachelor's degree, CCM certification and six months to one year of home health experience are preferred as well. You must have reliable transportation and a valid driver's license with a good driving record. All new hires will be required to undergo a criminal background check and provide proof of a current CPR certification.
