VNSNY’s core services include skilled nursing, rehabilitation therapy, medical social work, nutritional counseling, and personal care services. These services are provided through a variety of programs: Acute/Adult Care Community Mental Health Congregate Care Early Head Start Early Intervention HIV/AIDS Services Home Health Intake Coordination Hospice/Palliative Care Long-Term Home Health Care Program Maternity, Newborn, Pediatrics Nurse-Family Partnership Program Infusion Care Services Rehabilitation Services VNSNY CHOICE/Managed Long-Term Care Visiting MD Program VNSNY CHOICE -an affiliate of the VNSNY-offers six health plan options to residents of New York City (Brooklyn, Bronx, Manhattan, Queens and Staten Island) who are eligible for Medicare, Medicaid, or both.
PURPOSE OF POSITION
Responsible for enrolling new members to meet established growth targets for VNSNY CHOICE Managed Long Term Care health plan through marketing program in a variety of venues. Promotes awareness, understanding and visibility of VNSNY CHOICE among potential members, and those with influence over health insurance plan selection.
RESPONSIBILITIES
- Develops and conducts marketing activities, such as visiting NYC CASA offices and local senior centers, and making presentations designed to promote VNSNY CHOICE to potential members, physicians and other community resources.
- Establishes new and maintains ongoing relationships with internal and external referral sources to promote future enrollments and ensure the continued growth of VNSNY CHOICE through new enrollments.
- Participates in hiring, promoting, terminating and recommending salary actions, as appropriate. Provides and explains information regarding the services provided by VNSNY CHOICE, including Medicaid and Medicare coverage, to referrers, potential members and families.
- Provides training for staff in other VNSNY programs about VNSNY CHOICE and related topics as requested. Conducts comprehensive clinical, psycho-social, and financial risk assessments of potential members for enrollment to VNSNY CHOICE across multiple regions. Evaluates appropriateness of membership based on NYS assessment criteria.
QUALIFICATIONS
Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required. Valid driver’s license and access to a vehicle is required for regions outside of Manhattan.
Education: Bachelor’s Degree in Nursing, or the equivalent required. Master’s Degree in Nursing preferred.
Experience: Minimum two years nursing experience in a Home Care or Managed Long Term Care Program, including one year conducting enrollments required. Strong knowledge of Federal and state health care regulations related to Medicare, Medicaid, managed care, and long term care required. Effective oral, written and communication skills required. Proficiency in personal computer operations including MS Office applications required. Bilingual skills may be required as determined by operational needs preferred. Knowledge of geriatric population preferred. Effective negotiation skills preferred.
