RN Field Care Advocate - Middle Tennessee Area
The Medical Home Care Advocate supports assigned practices with on site coordination of care of cohorts of high risk members to ensure appropriate primary care follow up and reduction in adverse events. The Care Advocate works with the provider care coordinator to assist with coordinating care with practice teams/resources and UnitedHealth care management teams/resources to ensure high risk patients receive optimal care at each visit to practice. Goal of the Care Advocate is to be a resource to the provider office and provide support for the medical home.
Primary Responsibilities:
- Works on site with assigned Medical Home practice clinical team to assist with management of all target group cohorts of high risk members and ensure they are seen by their PCP as needed to meet goals
- Work directly with the PCP Care Coordinator and PCP
- Coordinate support for patients using available United care management resources
- Collaborate with the Medical Home clinical team
- Report daily, weekly and monthly progress to clinical teams
Required Qualifications:
- Active, unrestricted RN license in Tennessee
- 3+ years experience as an RN in any clinical setting
- Ability to travel within a 60 mile radius of Nashville, TN, with a reliable vehicle and appropriate insurance
- Proficient level of knowledge of Microsoft Word and Excel, with the ability to navigate a Windows environment
Additional Preferred Assets:
- Experience working within a Private Practice setting
- Prior Case Management experience
- Certified Case Manager (CCM)
- Prior experience working with the Medicaid population
- Bilingual in Spanish
