RN – Home Health Nursing (Full Time
Sutter Senior Care is an affiliate of Sutter Health, a not-for-profit, community-based health care network in Sacramento, California. Senior Care is a Program of All-Inclusive Care for the Elderly (PACE). This comprehensive health plan includes a broad range of health care and support services that enable frail adults to remain in their homes rather than enter institutional long-term care.
The PACE model was pioneered by On Lok Senior Health Services, which developed the first PACE program in 1983. Today, there are almost 30 PACE programs nationally and four in California. Research has shown that PACE providers are successful in reducing hospitalization and nursing home utilization. PACE providers have been acknowledged for their success in enhancing the quality of life of the frailest of adults and disabled older adults.
Job Summary
The Registered Nurse in the Home Health setting manages the patient/family experience through utilization of critical thinking, teamwork, effective communication, and nursing judgment/knowledge/skills.
MINIMUM POSITION REQUIREMENTS:
Education, including Licensure, Certification, Registration
Must have reliable automobile and Valid Auto insurance required. Current licensure California RN or Interim Permitee; Must have BLS -Health Care Provider. Graduate of an accredited school of nursing; BS preferred.
Knowledge
Must have demonstrated knowledge of basic nursing skills and effective English written and communication skills.
Knowledge of population-specific learning principles relative to patient teaching.
Experience
Minimum one (1) year experience in Home Health required; 2-5 years experience preferred. One year (1) experience working with elderly required. One (1) year minimum of acute hospital experience (adults) or SNF preferred.
Prior experience working in discharge planning and/or outpatient setting. For Trauma:Emergency or critical-care experience. Outpatient or clinic experience to include knowledge and use of available resources and systems.
Skills
Independently practices and prioritize multiple demands in a ever-changing environment. Good interpersonal, communication (both oral and written) and teamwork skills is required.
