RN - CLINICAL COORDINATOR - FULL-TIME BENEFITED; SALARIED POSITION
RN, BSN with a minimum of an Ostomy Certification will be considered
Position Highlights:
The Wound and Ostomy Certified RN expertly provides and coordinates care for patients with wound, skin and ostomy needs that include assessment, planning, intervention and evaluation. The certified RN promotes evidence based patient care through the integration of research and quality improvement strategies in clinical practice and provides patient care in an ambulatory care setting. The nurse works in a collaborative relationship with other members of the health care team to provide leadership and to coordinate the care of patients. Principal Duties and Responsibilities
- Performs and documents wound and skin assessments using a comprehensive and systematic approach.
- Formulates an evidenced based plan of care and communicates plan to nursing, physicians and other health care providers as appropriate.
- Communicates ongoing wound, skin and ostomy care needs and interventions across the care continuum.
- Develops, conducts and participates in formal and informal education of patients/families and staff in area of expertise.
- Contributes to development of policies, procedures, protocols and/or patient educational materials as appropriate in area of expertise.
- Provides guidance and expertise for product usage and techniques.
- Provides telephone triage regarding symptom assessment, information, intervention and appropriate Physician referral for patients.
Knowledge, Skills and Abilities:
- Current RN Licensure in the State of Illinois
- Bachelor's degree in Nursing from an NLN accredited school of nursing.
- Minimum an Ostomy Certification; prefer Wound and Incontinency Certification
- Progressive experience and advanced knowledge in skin, wound and ostomy care.
- Certification in wound and/or ostomy nursing from an accredited program
- Current CPR Certification
- 1-2 years nursing experience required
- Basic computer skills required, Epic or EMR experience preferred
- Good communication and interpersonal skills
- Ability to organize, and be self-motivated
