Deer Meadows Retirement Community in Northeast Philadelphia is looking for a full time Social Service Worker. The SSW provides a diverse range of clinical Social Work interventions and services to residents, families, and others. Usual schedule Mon-Fri 9 a.m. to 5:30 p.m, some flexibility in scheduling may be required. Requires BSW or bachelors degree in a related field with strong customer service skills, prior experience in a health care setting preferred. Applicants are encouraged to have knowledge of MDS 3.0 and strong skills in planning and organization are essential. Position also requires familiarity with discharge planning and community support resources as well as sensitivity to resident and family needs. In addition computer skills and experience with Microsoft Office are necessary. Knowledge of Medicaid/Medicare policies and procedures relating to long-term care are also a plus. Caseload averages 70 residents, a portion of which reside on our respiratory care unit.
