Social Work Supervisor
As the Supervisor of Social Work you will supervise professional social work staff, perform psychosocial assessment of patients and families, formulate and implement psychosocial treatment plans. You will also identify unmet psychosocial needs and develops programs to meet those needs and participate in educational activities, professional organizations and hospital committees. Responsibilities Include: Supervises the professional social work staff. Assists in screening applicants for Social Work positions. Meets with staff social workers regularly to review caseloads. Monitors time, charting and statistics of social work staff in accordance with departmental criteria. Demonstrates knowledge of standards of professional practice and assists staff in examining clinical, administrative and ethical issues. Performs timely competency assessment/performance appraisals according to institutional schedules and policies. Performs psychosocial assessments of patients and families. Identifies patients needing psychosocial services. Participates in scheduled rounds. Meets with staff to discuss and receive referrals for early psychosocial intervention. Interviews patients and families, when necessary, in order to assess family structure, dynamics, adjustments to illness and access to necessary resources. Educates patients and families about community resources and discharge and planning procedures. Identifies medical problems and predicts course. Formulates and implements psychosocial treatment plans. Uses clinical skills in counseling individuals and families dealing with illness, disability, hospitalization and medical treatment. Facilitates appropriate referrals to community resources and facilities for post-hospital care. Communicates with community agencies to coordinate medical and psychosocial services, and to provide information to other health care facilities to facilitate referral. Assists families in understanding and negotiating community resources, especially post-hospital health care facilitates. Defines community services to hospital staff to facilitate referrals. Participates in interdisciplinary treatment planning conferences. Maintains all relevant documentation. Additional Requirments / Information
