SOCIAL WORKER
SJLC is a 120 bed Catholic, not for profit skilled nursing facility; comprised of an active short term rehab unit, as well as long term care units. An opportunity exists for a full time (32-40 hours per week) Social Worker to assist in assessment, care plan development, discharge planning needs for a diverse population- while understanding the mission of the organization. Educational requirements are a BSW required; MSW preferred. Exposure to long term care and experience in geriatrics is preferred.
