Sr Manager, Clinical Program Mgmt
This role will provide day to day client leadership and project management to support Accountable Care clients. This individual will be focused on providing solutions to hospitals, providers and health plans that are forming new integrated care models to drive industry leading outcomes and cost trend management. The Account Manager will own the day to day client relationship, develop the implementation plan, and project manage various implementations. This individual will collaborate closely with the various CVS Caremark businesses. The role will develop an intimate understanding of the objectives of the customer and various participating stakeholders with the goal of establishing a suite of services with the customer. Services may include: ?Engagement in re-admission prevention programs, both home-based pharmacist programs based in the PBM, as well as ambulatory programs in retail locations with MinuteClinic NPs and CVS Pharmacists ?Discharge medication programs in hospitals based on CVS pharmacist visits, prescription provision, and counseling ?Enhanced clinical programs with MinuteClinic that would address unnecessary Emergency Department utilization by providing an alternate site of care for basic acute medical problems ?Chronic disease monitoring visits and services provided at MinuteClinic, including point of care laboratory testing as indicated ?Biometric screening opportunities at MinuteClinic ?Pharmacy programs targeted at generic switching at retail locations ?Comprehensive vaccination programs for health system patients at retail pharmacies and MinuteClinic ?Wellness offerings to address obesity and smoking cessation at MinuteClinic among others
