Support Group Facilitator
Under general supervision of program coordinator, conducts support groups for Kaiser Permanente Bariatric Surgery Program members. Meets with coordinator and other support group facilitators on a regular basis. Uses Kaiser-approved written resources to provide accurate information to support group members. Provides ongoing support to both support group members and other support group facilitators. Makes recommendations to coordinator for improving services to program members. Essential Functions: - Maintains understanding of Kaiser Bariatric Surgery Program, processes and protocols; attends regularly scheduled monthly facilitator's meetings; conducts sessions and reflects understandings by relaying current and accurate program information to group members - Coordinates logistics by assisting in securing meeting sites and dispersing information to group members - Keeps accurate attendance records for all group sessions; forwards records to program coordinator at Kaiser Bariatric Surgery Program office after each session - Demonstrates understanding of facilitator's role; limits self-disclosure of personal information as appropriate; follows Kaiser and departmental policies and protocols to function within the scope of this position - Facilitates group process by ensuring every group member has an opportunity to share thoughts and feelings; ensures group environment remains safe and confidential for group members; maintains confidentiality as a group rule and models group value of respecting privacy and needs of individual group members - Informs program coordinator of any concerns arising in group or in facilitation relationship - Discourages group members from endorsing or promoting any health claim, product or information (e.g., drugs, potions, programs, etc.) that has not been explicitly endorsed by Kaiser Bariatric Surgery Program - Facilitates communication with members and providers in Bariatric Surgery Program; encourages members to contact appropriate provider in problem areas; ensures members have access to services by informing program coordinator if a group member is not being served in a timely manner; acts as liaison between group members and Kaiser Bariatric Surgery program via communication with program coordinator; informs coordinator of any group suggestions for program improvement - Performs routine documentation, tracking and data collection as required; completes administrative duties as required - Orients new members to Kaiser Bariatric Surgery Program; explains group rules and introduces new members to group; supports group sessions and lifestyle classes in other Kaiser departments by directing group members to Kaiser's wellness calendar as appropriate - Attends ongoing educational sessions to maintain and increase knowledge on evidence based weight management strategies - Keeps informed of new developments in area of weight management - Demonstrates sufficient knowledge, skills, and abilities in area of specialization - Demonstrates knowledge, skills, and abilities necessary to provide care and/or service appropriate to age groups served - Performs other duties and accepts responsibility as assigned Qualifications: Basic Qualifications: - Counseling, group facilitator, or related experience - High school diploma - Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification - Demonstrated knowledge of and skill in adaptability, change management, decision making, customer service, group process facilitation, interpersonal relations, oral communication, teamwork, written communication, diversity, and empathy - Talking to co-workers, customers, outside vendors, and on the telephone - Reading, writing, speaking, and understanding English - Training/giving and receiving instructions - Mathematical ability, attention to detail (e.g. organization, prioritization, proofing), concentration, and alertness Preferred Qualifications: - Participant in Kaiser Hawaii Bariatric Pre-surgery Program - Completed surgery with Kaiser Hawaii Bariatric Surgery program three months or more - Knowledge and understanding of Kaiser Bariatric Surgery program processes - Post high school coursework in psychology, sociology, counseling, mediation, or related field
