Supported Living Case Manager Job in Canton 02021, Massachusetts Us

Networks, a program of Enable, Inc., is seeking a full-time Supported Living Case Manager.  Working as part of an energetic, creative and dedicated team, the Case Manager assists individuals to live as independently as possible in their own homes.  The Case Manager will work with individuals who have physical disabilities and co-occurring disabilities. 

 

Candidate must have knowledge of rehabilitation practices and the needs of individuals transitioning to independence.  Ability to combine education in community living with provision of direct service essential.  Residential services or rehabilitation services background preferred.  Sense of humor and dedication required.  BA/BS and 2 years experience acceptable; MA/MS/MSW and 1 year experience preferred.  Travel required; must have own car.
