Provider Relations Coordinator, HouseCalls Job Description: POSITION SUMMARY The Provider Relations Coordinator works closely with the Regional Lead Coordinator and the network management team to manage a regionally based team of practitioners to complete in home health assessments (i.e. HouseCalls).
